Birth certificate (original or certified copy with raised seal)
Social security card (NOT LAMINATED)
Parent whose name is listed on the birth certificate or legal guardian with court ordered guardianship papers, MUST be present.
Parent or guardian must present a valid form of ID
ABOUT THE KENTUCKY CHILD ID:
On January 2, 2003, Kentucky implemented a new "CHILD ID" program for applicants up to 15 years old. This program works closely with the Kentucky Stat Police "MISSING PERSONS CLEARINGHOUSE" and the AMBER ALERT system in the event of a missing child.
One notable difference on the CHILD ID is that the child's address and signature will not appear on the face of the ID card. The address is stored int ehe Kentucky database but does not appear on the card to protect the child in case the card is lost.
